Dark Side Creations accepts Visa, Master Card,American Express, cheques and
 money orders.

Prices listed on this Order Form are for designs manufactured in any of our 9 Standard color options.
Black, Red, Green, Purple, Pink, White, Blue, Semi-transparent, and Yellow.
Metallic and Iridescent effects are available in all of our Standard color options.(Plus Metallic Silver)
A 15% customizing charge will be applied for Metallic effects; a 20% customizing charge will be applied for Iridescent effects.

 
-Prices are listed in Canadian dollars.
-All orders are pre-paid.
-G.S.T. at prevailing rates will be charged on all products sold within Canada.
-Shipments are FOB Origin (Calgary), Freight Prepaid & Add
–No merchandise will be accepted for return unless specific authorization is given in advance.
-Dark Side warranties our products for 90 days.  Our liability is limited solely to replace or repair merchandise.

U.S. Credit Card Customers:
Although your financial institution will determine the exact exchange rate, 
divide the Canadian total by 1.6 to determine the approximate amount of your order in U.S. dollars.
U.S. Money Order/Certified Check Customers:
Divide the Canadian total by 1.3 to determine the total to remit in US dollars
